In Morocco, carbapenems are the last line of antimicrobial therapy against Enterobacteriaceae extended-spectrum beta-lactamases producing (ESBLs). The resistance to this class of antibiotics is increasingly recognized causing real therapeutic impasses. In our context, this is more experienced. The objective of this study is to report the experience of our microbiology laboratory to detect Klebsiella pneumoniae resistant to carbapenems between March 2011 and March 2015 Identification of Enterobacteriaceae is based on the Biomerieux API 20 E system, susceptibility testing is performed by disk diffusion method and resistance to imipenem suspected and confirmed by the modified Hodge test and measurement of the MIC by E-test method. During these two years, the total number of reported ESBL Klebsiella pneumoniae was 73 isolates. 16 strains were suspected carbapenemase-producing (21, 9 %) and confirmed for only nine patients (12.3%). The average age of our patients was 64.75 years. All were hospitalized and monitored for uro-nephrology. Patient treatment was proposed following the alternatives revealed by testing susceptibility, generally amikacin and colistin. The outcome was favorable in seven cases. Care facilities are now threatened by the expansion of this new mechanism of resistance. Thus, it is recommended that all K. pneumoniae isolated from infection should be suspected carbapenemase-producing if it proves multi-resistant. The susceptibility to carbapenems is to be identified by means of simple tests such as the modified Hodge test and synergy with EDTA.
