Ebstein anomaly is a rare congenital heart malformation typically involving the tricuspid valve and the right ventricle. It has a wide range of anatomical and pathophysiological presentations. In adults, the most frequent manifestations are arrhythmias. The prognosis mostly depends on the functional right ventricle size. We report the clinical case of a 30-year-old woman with no medical history who was admitted for the management of palpitations. The electrocardiogram found tachycardia with positive Delta waves, transthoracic and transesophageal echocardiography showed severe Ebstein anomaly with a major atrialized right ventricle. After cardiac catheterization and echocardiography, a single-ventricle repair was proposed with a tricuspid valve repair and a partial bidirectional Glenn procedure.
