Pleomorphic adenoma is the most common benign tumor of the salivary glands, with a predominant occurrence in the parotid gland. Its manifestation in the submandibular gland is uncommon, posing both diagnostic and therapeutic challenges. Clinically, it typically appears as a slow-growing, painless mass in the cervical region, often contributing to delayed diagnosis. Imaging is pivotal for characterizing the lesion, evaluating its local extent, and aiding in preoperative planning. A definitive diagnosis is established through histopathological examination. The treatment of choice is surgical excision of the submandibular gland to minimize the risk of recurrence or malignant transformation. We report the case of a 28-year-old woman who reported a painless, progressively enlarging swelling in the left submandibular region. Clinical evaluation and imaging findings suggested a benign salivary gland tumor. The patient underwent surgical removal of the submandibular gland, and histopathological examination confirmed the diagnosis of pleomorphic adenoma. The postoperative recovery was smooth, with no signs of recurrence observed during follow-up.
