KITE V/S PONSETTI TECHNIQUE FOR SERIAL MANPULATION AND CAST APPLICATION IN TREATING IDIOPATHIC CLUB FOOT

Abstract KITE VERSUS PONSETTI METHOD FOR MANIPULATION AND SERIAL CAST APPLICATION IN IDIOPATHIC CLUBFOOT INTRODUCTION :- In the Kite method, the fulcrum is cuboid and in Ponsetti method the fulcrum is head of the talus.The Kite method of serial manipulation and casting achieved success rate ranging from 11% to 58 % in literature.The Ponsetti method achieved success rate from 78% to 89% in various literature AIMS & OBJECTIVES :- The goal of the treatment to achieve a pain free,plantigrade foot with good mobility and joint function. MATERIALS AND METHODS :-10 bilateral cases – 5 patients had both feet with Kite’s method , 5 patients had both feet with Ponsetti method RESULTS:- Mean age of treatment – 95.7 days (3- 180 days),More common in male child (76.2%),57.89% – Bilateral,Right side (68.75%)> left side,Association with DDH – 1 case,90% had full term normal delivery,None had positive family history. 10 bilateral feet , Ponsetti method showed faster rate of decrease in Pirani Score (Rate of Improvement) than the Kite method , Hind foot score, midfoot score and total score, From 0 through 4th week upto 10th week, Statistically significant difference (P<0.05) DISCUSSION:- Idiopathic club foot incidence – 1 to 2 per thousand births, The intention of the study analysis compares two groups formed randomly, To minimize the bias POSITIVE POINTS OF THE STUDY ARE :- Unique study, Previously done by only 1 group, Randomly distributed patients, Feet were compared among two groups in same patient, Standard scoring system, Consistent with the previous literature LIMITATIONS OF THE STUDY :-The group is small, Can’t be extrapolated for a larger population, Still the results from other groups needs to be understood, Surgeons skill bias CONCLUSION:- Ponsetti method is better than Kite’s method in achieving results in patients with congenital club foot
