Caustic detergent ingestions, intentional or accidental, remains a significant health hazard in underdeveloped parts of the world. We came across an unusual presentation of an elderly male with accidental detergent soap-bar ingestion who developed immediate oropharyngeal ulcerations. Although he did not have any immediate esophageal or gastrointestinal involvement, he developed chemical pneumonitis, which required hospital stay with conservative management. This case highlights the features of pulmonary involvement indetergentsoap-bar ingestion in a small amount and the value of aggressive emergency and intensive management which can result in a favorable clinical outcome. In our knowledge and subsequent literature searches, this is probably the first case report of this unusual presentation.
