Liver lipoma is an extremely rare tumor. It is usually an asymptomatic lesion diagnosed incidentally on abdominal ultrasonography (US) imaging for nonspecific abdominal pain. This exam (US) objects a well demarcated homogeneous hyperechoic lesion which is not distinguishable from other hyperechogenic liver lesions such as hemangioma or metastasis. Doppler does not provide additional information. The differential diagnosis is particularly important for patients monitored in oncology, as well as ultrasound, other imaging modalities must be performed namely CT and MRI which are useful for identifying its pure fat content, saving the recourse to biopsy remains an invasive examination. We report a new case of a incidental liver lipoma in a 60-year-old patient and an abdominal ultrasound and confirmed by MRI
