Abstract
Introduction: La coronarographie reste l'examen de r?f?rence pour l'?tude de l'anatomie coronaire, mais l'?lectrocardiogramme refl?te la physiologie du myocarde au cours de l'isch?mie aigu? permettant ainsi de reconnaitre la l?sion coupable. Le but de ce travail est d??tudier la corr?lation entre les donn?es ?lectriques et coronarographiques et de juger la reproductibilit? de l??lectrocardiogramme dans l?identification de la l?sion coupable.
Patients et m?thodes: C'est une ?tude r?trospective ?tal?e sur une p?riode de trois ans (2014 ? 2015 ? 2016), incluant patients qui sont admis au service de cardiologie de l'h?pital militaire Avicenne de Marrakech pour un syndrome coronarien aigu ST (+) et qui ont b?n?fici? d'un examen cardio-vasculaire, un enregistrement ECG de 18 d?rivations, d'un bilan biologique, d'une ?chocardiographie et de la coronarographie.
R?sultats: Les IDM ant?rieurs repr?sentent 62,5 % des cas de l'ensemble des IDM de notre ?tude. L'IVA est l'art?re responsable dans 73,5 % des cas. Le sus d?calage du segment ST est retrouv? majoritairement dans les d?rivations V1, V2, V3 et V4. L?infarctus ant?ro-septal est d? ? une atteinte de l?IVA moyenne dans 58,3 % cas, par contre l?infarctus ant?rieur ?tendu est en rapport avec une atteinte de l?IVA proximale dans 71,4 % des cas. Les l?sions associ?es en cas d'IDM ant?rieur: La coronaire droite dans 28,57 % des cas et la circonflexe dans 7 % des cas. L?image en miroir dans le territoire inf?rieur a ?t? observ?e dans 30 % des cas. Les IDM inf?rieurs repr?sentent 35 % des cas. L?art?re coronaire droite est responsable de l?infarctus dans 64,28 % des cas. L?art?re circonflexe est l?art?re coupable dans 28,57 % des cas.
Conclusion: L?ECG reste un outil incontournable dans l'identification pr?coce de l'art?re responsable de l'infarctus afin de guider le cath?tiseur ? chercher et ? identifier facilement la l?sion coupable ? la coronarographie.
