LA LEISHMANIOSE VISC?RALE AU COURS DU LUPUS ?RYTH?MATEUX SYST?MIQUE, UNE NOUVELLE OBSERVATION AVEC REVUE DE LA LITT?RATURE.

Abstract

La leishmaniose visc?rale est une forme particuli?rement grave de l?infection par les parasites intracellulaires du genre Leishmania. Bien que c?est une affection immunod?primante, la survenue de la leishmaniose au cours du lupus ?ryth?mateux syst?mique est rarement d?crite. Nous rapportons le cas d?une patiente suivie pour lupus ?ryth?mateux syst?mique, qui pr?sente un tableau de fi?vre prolong?e nue avec discr?te spl?nom?galie. Le bilan biologique confirme la pouss?e de la maladie lupique avec un syndrome inflammatoire sans s?rite. Malgr? un traitement cortico?de et immunosuppresseurs, l??volution est marqu?e par l?aggravation de l??tat g?n?ral, la persistance du syndrome f?brile et l?apparition d?un syndrome d?activation macrophagique. Le m?dullogramme montre des formes amastigotes de leishmania sp. La patiente est trait?e par antimoniate de m?glumine avec bonne ?volution clinique et biologique. Cette observation pose le probl?me du diagnostic de la leishmaniose visc?rale au cours du lupus ?ryth?mateux syst?mique du fait de sa pr?sentation clinique souvent atypique. Le diagnostic doit ?tre ?voqu? devant tout tableau infectieux prolong? inexpliqu? chez un patient atteint de lupus ?ryth?mateux syst?mique m?me en absence de s?jour en zone d?end?mie.
