Abstract
Background: Rudimentary horn pregnancy is a rare ectopic implantation associated with uterine rupture and potentially fatal intraperitoneal hemorrhage. Ultrasound diagnosis remains challenging, particularly in asymptomatic patients. Case presentation: A 27-year-old woman, gravida 4 para 2 with one previous miscarriage, underwent ultrasound during a medical outreach visit in Tidjikdja, Mauritania. The scan showed an empty uterine cavity with thickened endometrium and a right parauterine gestational sac containing a nonviable pregnancy estimated at 17 weeks of gestation. Both ovaries were visualized and no fluid was seen in the pouch of Douglas. The patient was asymptomatic and hemodynamically stable. Her abdomen was soft and non-tender, with a painless right-sided hypogastric mass. Because an advanced ectopic pregnancy was suspected, she was transferred to the National Hospital Center in Nouakchott. Laparotomy confirmed an unruptured pregnancy in a right rudimentary uterine horn. Excision of the horn and ipsilateral salpingectomy were performed. The postoperative course was uncomplicated.
